Don't force it: Discuss the reasons why the person is not wanting to have an exam.  It could be fear or having had a bad experience. You could find a doctor that might be a better fit.  You can not force the person.

it may be a phase.  Cetainly there are medical causes, but he may have an intrinsically high metabolic rate.

See who you wish to: America is a wonderful country and a free country. That means each person can go to see whichever doctor she wishes to see as long as she is paying for her own medical care. If she is having someone else or some insurer pay for part or all of her medical care, then that other person has some say in the matter. In this free country, doctors may refuse to see a patient, with some exceptions.

Usually, a doctor will continue helping a patient for a month, while the patient transitions to her new doctor, unless the situation has some special circumstances. One can pre-pay the old doctor to send by fedex her records to the new doctor.

That : That is a difficult question.  Osteomyelitis is a very rare condition.  It is very difficult to diagnose by examination.  The bacteria grow slowly in the bone, so it is hard to know if the area would have been seen on an MRI four weeks prior.  It would be interesting to know if you had a fever at the original appointment or if you have a history of past spinal procedures or risk factors for osteomyelitis.  If you had new severe pain, fever, history of past spinal procedures, risk factors for osteomyelitis, or neurologic changes like leg weakness then an MRI would have been appropriate.

Not illegal: Your doctor may be concerned about the "complex back pain" you have listed as a clinical finding,  and is trying to understand all possible information about this.  S/he may wonder about you being on medicare at a young age (if you are).  S/he may refer to to a psychologist to help any stress/emotional issues which can contribute to pain, and may decide not to see you if you won't work on this.

In fact, most successful treatment of agoraphobia takes place outside of the "office." i would encourage you to find a specialist, particular a licensed psychologist, who outlines such treatment.
