Is it painful?: Not every documented rotator cuff tear is painful so surgery may not be indicated. Has a conservative approach been tried e.g. Rehab for two or more months? You are basically describing one tear. Discuss with your orthopedic surgeon your options.

Ultrasound: After a course of physical therapy I usually recommend a dynamic ultrasound evaluation to assess the integrity of the tendon.

Depends: Surgical treatment for full thickness rotator cuff  tears (RCT) depends on age, activity level, pain level, weakness, and tear characteristics (size, location, amount of retraction, chronicity).  Your orthopaedic surgeon will discuss these issues to help you decide if and when surgery is appropriate.  33 is very young to have a RCT and I usually recommend surgery for acute tears at this age.

how long does it take to strengthen your rotator cuff muscles after having rotator cuff repair surgery for a massive tear where tissue is good quality?

Patience: It can take 12-24 months to maximize strength gains after a large repair, providing the tissue heals correctly

pls advise which drugs can be used for a rotator cuff tear which does not require surgery. pain is moderate but ongoing, have full movement/strength ?

Nsaids?: Im not sure what you mean by a rotator cuff tear that doesn't require surgery.
But at age 74, I would certainly try to decrease your
pain and swelling with some nsaid medications
Then Id work on improving your range of motion and
shoulder strength with specific rotator cuff exercises

Yes....Maybe: It depends on how old the tear is and the quality of the muscle(fatty infiltration).

Conservative: Unpredictable , but high chance if you had good physical therapy results and continue  exercise you can postpone surgery indefinatly.   Talk to ortho surgeon to review all options.

Be seen: I would certainly let your surgeon know as there may be a concern for a repeat tear of the rotator cuff tendons.
