Yes: It usually occurs because something is placed inside the urethra and pulled out roughly, tearing the opening.  Sometimes piercings can be pulled loose and tear the opening.  (especially, prince alberts.).

i have a few horizontal tears on the glans as a result of dry masterbation (i don't suspect std's). how could i take care of these tears? there is also a small scare on the meatus that is not closing?
p.s. my urologist said its nothing important

: Good new if your urologist says it is nothing important!  You can usually treat small tears with just topical antibiotic ointment for a few days and give the tears a chance to heal for 5-7 days or so.

suffered a tear on my penile opening, otherwise known as the meatus it is vertical going down the frenulum and is probably the width of a paper clip (not big at all) i feel no pain whatsoever. but am wondering if it will close itself up with time?

: Minor tears like that are not unusual and they self heal over time.

Urethritis: Infections inside the urethra are most often either chlamydia or gonorrhea.  Yeast does not cause the symptoms of urethritis. But, yeast infections are common on the skin around the penis and scrotum.  

Perhaps your local std clinic or a good men's health expert can help you sort this out.

meatus of penis is bigger, maybe from rough masturbation. used to be small slit, but is now open like lips. urine flows inconsistent too, sprays sometimes and leaks after. will urethra heal/go back to original size? could it be scar tissue/blockage?

Very possible: If so, probably so since birth. Name of condition is congenital hyperspadias. When it is below midline on penis (much more common) then it is called hypospadias.

: Because the penis is a shape fluctuating organ, it is normal for the meatus to look different depending on erection, partial erection, even just which way it has been laying within your underwear. This is normal and not any reason for concern. Use a condom every time for safe sex and contraception.

: It could be local irritation which sometimes occurs in uncic. men.  Keep penis foreskin clean and apply OTC cortisone cream. If not better the pl see a Urologist for without exam it is not possible to be exact. Good luck

: Any urinary symptoms? Was a microscopic examination of the urine sediment performed to rule out urethritis? If not, I highly recommend one be done.   If inflammation of the urethra is not an issue, one wonders whether very tight jeans, bicycle riding or any other athletic activities could be a cause. Wear loose trousers, white cotton boxer style underwear. If no urologic cause, see dermatologist.
