can hives appear in the same  exact spot every time?

Usually not: Hives tend to come and go in random areas.  There is something called dermatographism which is a hive-like condition that appears when the skin is irritated or stoked.  Dermatographism can appear in the same place over and over again if the irritation is repetitive. Treatment includes anti-histamines like Allegra (fexofenadine) 180 mg once daily.

if you were to get hives from reacting badly to medicine, do the hives have to appear all over the body or can they be in a tiny spot only?

Hives from: a medication allergy are generally all over (and can be dangerous if this swelling occurs in the upper airways!!)

Cellulitis: Either  got  infected  or   inflmation  got  deeper   involving  hair  follicle  or  muscle.

Contact Dermatitis: Allergic contact dermatitis (ACD) localized to the site of ink injection are often associated with metallic salts used in tattoo pigments (eg, mercury, chromium, cadmium, cobalt, nickel). Treatment is avoidance and treatment of skin inflammation. Topical or oral corticosteroids may help alleviate symptoms. Ultimately, removal of tattoo using laser surgery should be considered.

Rash: Hives usually present as red, raised, itchy lesions that can be small or large. "Hives" are often used non-specifically to describe many different skin lesions so I would have your rash looked at directly in order to make the correct diagnosis. Hives do tend to itch and individual lesions usually appear and resolve completely within 24 hours. See your doctor for further treatment and evaluation.

Not likely: The timing does not favor implantation bleeding as that would be expected in mid-cycle. Implantation is unlikely three days following a period.

Hives: It sounds as if you are having a dermatologic condition called urticaria.  Urticaria is the medical terminology for hives.  It is the bodies response to a certain stimulus, such as an allergy.  The body will release histamine in response to a stimulus and histamine causes redness, swelling and itching.  Other triggers of urticaria can be cold/heat, stress, medicines, or with you, pressure on skin!
