What is wrong: hip?: Treatment depends on what is wrong with your hip.  You need a basic ap-pelvis so both hips can be seen.  Before this, a thorough exam. Patients frequently think sij pain, pyriformis syndrome, and trochanteric bursitis is hip pain.  They are injected differently.  Pain on internal rotation of the hip is the best clinical finding to suggest hip pathology. Apart from severe osteoarthritis, i inject!

Was more common..: Less common today, since some studies cast doubt on serious benefitss.  In my experience they appeared to help.  However, their is no coding specific for facet blocks, so that doctors can bill. These are called cpt codes and facet injection was removed 1/1/11. Nonetheless, a common procedure, which if done properly will not hurt.

Location: Facet injections place anti-inflammatory steroid into the joint(s) in the spine and the epidural steroid injection places steroid around the spinal cord, nerves and discs. 
Joint injection is very localized and usually requires more than one. ESI can have greater spread. Since the epidural space is just outside of the spinal cord, this is more risky. Either injection should be done under XRAY.

Probably: You are at an increased risk of infection overall while on remicade (infliximab). Using a strict sterile technique and wearing a surgical mask as a doctor can decrease this risk substantially but it will not be 0. Talk to your pain physician about it.

Back pain: The source of the pain may be from a pinched nerve or a tear in the disc rather than inflammation in the facet joint. Alternatively, the facet joint may be too degenerated to respond to cortisone shots alone. Back bracing, physical therapy or chiropractic care may relieve the pain. If these measures fail to eliminate the pain and then surgery may be the next option.

how effective are ozone injections to relief pain caused by kyphosis and facet pain? do they have side effects?

Ozone: It has shown some promise with herniated discs but not for the treatment of kyphosis or facet pain.
