: Due to the fact that you are noticing ear symptoms along with dizziness and balance issues when you walk, this could be an inner ear issue that is related to the blocked feeling he have in your ears. A face to face examto look at your ear canal, eardrum and assess for a possible infection that is behind the eardrum would be very helpful. Treatment for inner ear issues include Epley maneuvers.

Though not : A prominent side effect it can happen both as an adverse reaction from the medication, but also from a rapid change in your blood sugar which is unusual but occurs.  Discuss with the prescribing doctor as this sounds severe for you.

Bleeding, anemia: Fibroids, also called leiomyomas are benign uterine tumors (not cysts) that can cause pain and heavy bleeding with your periods. If they are causing very heavy periods, this can lead to anemia and the dizziness, lightheadedness, etc you described. They are easily diagnosed with an ultrasound. See your ob/gyn for this.

Need to be seen: The cause could be something benign like orthostatic hypotension but could also be something more serious. A very good history is important in deciding how likely it is to be serious and what other tests need to be done. An exam provides additional information and tests like echo, ekg or head imaging may need to be done. Make appointment to be seen soon.

Maybe benign: Although migraine syndromes can include problems with dizziness and changes in vision, the loss of balance is concerning, as this could indicate inner ear or brainstem problems.  Since this could be associated with anemias, medication rxtns, and thyroid problems, or even sinusitis, best to get examined and diagnosed.
