Surgical diversion: I am not a surgeon, but it would seem reasonable to consider undoing the diversion if possible, to reduce the colitis. Long term implication would be a long-term need for a colostomy if things go southwards in terms of health. You need to discuss this in serious terms with your surgeon

Urethra still patent: Unfortunately closing off urethra or bladder neck is both difficult & has a high failure rate. Virtually impossible to do from below & best performed suprapubically by open surgery from above. Bladder neck has to be inverted & closed in 2 layers. Possibly your urologist could inject coaptite around bladder neck cystoscopically via the mitrofanoff channel using a pediatric cystoscope.

As long as you: have normal erection and sexual desire, and can actively participate in your boys' life, you should be do just fine.

Need to find cause: To treat any condition have to find the cause, if recent surgery will take some time for  the body to adapt, unusual to have pain, . You need to see your surgeon.

Whether a thing is big or small depends on what it is, what it's doing there, whether it's growing or shrinking, whether it's getting in the way of something else, etc... One's primary care doctor who knows the whole story, can best help figure things out. Details matter! For a nodule in the body, 1.9mm may be small whereas 1.9cm may be big.
