had inguinal hernia mesh repair 15ish years ago. still to this day if i get aroused but dont ejaculate i get a growing throbbing pain in and around testicles and lower abdomen. pain tends to last days. no other pain from site otherwise. insight?

Migraine headache is the most common throbbing head pain, trigeminal neuralgia, tension headache, and headache related to the sinuses are some of the other possible causes.  You may need an examination and testing to get the answer.
