Much liquid intake: Your intake of liquid exceeds your dialysis removal amount. You need to drink less or remove more liquid or both. Also, low salt diet makes your life easier since you don't get thirsty much a matter that makes you exceed your limits of intake.

Usually not: In pd the volume of fluid during each exchange is 2 liters and most patients are comfortable with that. We may use 1.5 l exchanges if someone feels swollen and doing pd in lying pistion sometimes help.

Ascites: Your pertoneal cavity is reaccumulating fluid. This may be secondary to an intraabdominal pathology or from low serum proteins seen in certain cases of chronic kidney disease. Your nephrologist can advise you better and would collaborate with the physician who performed the paracentesis to see if a traumatic process played a role in the fluid reaccumulation in which case the fluid may be bloody

Dialysis and BP: Your brother sounds acutely ill. He is on 6 medications, is his BP controlled? I would assume not. The fact that he is edema is his legs is the reason that his physicians want to have him start dialysis. Did they mention his renal function. Perhaps he has renal failure, due to poorly controlled bp,  and he needs to start dialysis for that. Ask his doctors why his BP is not controlled on 6 meds.

Hard to say: It is hard to diagnose you without more info.  Where on the back?  Are the swollen areas itchy?  How many are there?  How long has it been there?  Are they red?

If the swelling is sudden and in only one extremity, a you should see your doctor immediately to rule out a blood clot.

One common site  is at the heel, particularly the achilles tendon.It is associated with hla b27 arthropathies like ankylosing spondylitis, psoriatic arthritis, and reactive arthritis.

Sacral swelling: Swelling in this area is difficult to predict. It could be a pilonidal cyst or a lipoma or an abscess or a malignancy . I would recommend a referral to see a general surgeon who would be able to take your history and examine you and make the diagnosis.
