Rotator cuff: Tear might be the issue or at least damage to the tendon. I would see an Orthopaedic doc and have them eval further with exam, MRI and possibly cortisone injection.

Just a bruise: Very likely just a bruise.  If you pinched your leg really hard, the same thing would happen.  Unless it gets all red around it and the redness continues to spread, don't worry about it - it will go away with time.

Get examined: This could be the residua of an old boil like infection, and it may be somewhat sensitive but should not be significantly tender. It's best to have your physician examine this area for you to be sure.

See your physician: I do not have a ready explanation. In the upper abdomen we think about the stomach and peptic ulcers as well as esophagus and or pancreas. The deep breath component may also suggest a pulmonary or lung problem. I would see a physician for a more thorough history and physical with possible diagnostic testing or imaging.

Bruise: For the first 24 hours, i would ice the effected area to minimize the spread of the bruise. After 48 hours, heat may be applied to allow the blood to disperse about the body. The amount of time it takes to heal is highly variable from person to person.

Wound care: Stop bleeding by applying pressure. Rinse wound w clean water. Clean surrounding skin w soap &amp; water on wash cloth. Remove debris w tweezers. If material remains in wound – seek medical care. Apply antibiotic ointment. Initially cover w bandage until some healing has occurred. Change bandage when soiled/ wet or at least daily. Need tetanus shot if you haven't had one in last 10 years, don't know
