Pictures may be more helpful..Having a an album that you can show the child pictures of mom regularly.

Second would be any food intolerance. See if there is any recurrent symptoms with certain foods offered? You might want to try rotating his diet.
Another would be parasites or bacteria in the GI tract.
What ever the cause, make sure he is well hydrated.

Rash: Herpetic Whitlow is a painful cluster of the small vesicles on the tip of the finger. Another possible cause of the similar rash is an  impetigo, or bacterial infection.
 In the case of dyshidrosis, there could be small vesicles under the skin of palms and fingers. It cause itching and the skin may became scaly. Dyshidrosis could be treated with the steroid creams.
