what does it mean when your head itches and you don't have lice/dry scalp or dandruff?

Itch: Itching scalp could be due to all the causes you listed, but it also could be due to a fungal infection.  The type of shampoo you use can also cause this so i recommend switching to a hypoallergenic product if you haven't already.  Also stress can sometime result in somatic symptoms such as itchiness, see your doctor soon to find the cause and get relief.

Dry Skin: There are a couple of reasons that could be responsible.  Dermatitis from change in shampoo.  Sunburn to  scalp.  Excessive use of hairdryer.

Lice can mimick: dandruff; however, direct observation using a magnified light will show moving parasites as well as eggs on the hair shafts, Dandruff will appear as oily/dry scalp skin and yes, both can occur at the same time. Please get examined.

Fungal Infections: One of the most common causes for scalp sores are ring worm on scalp otherwise called Tinea capitus. Bacterial infections can also cause sores on the scalp

Contact Dermatitis: Use of some hair products can provoke itching even without a rash. Changing to hypo-allergenic shampoos and conditioners may help.

Itching scalp: You need to see a doc to examine your scalp and  find a cause, however, you might try an antihistamine like benadryl (diphenhydramine)  25 mg tree times daily for a week. This will help control the itch no matter what the cause.  Possible causes include allergy to soap or shampoo, sensitivity to hair products, seborrhea or psoriasis , and many more.

See Allergist: Given you unbearable itching symptoms and rash refractory to topical steroids I would suggest making an appointment to see an allergist for further workup/evaluation. Eczema, contact dermatitis, urticaria (hives), etc. are all skin condition that you could be suffering from. Getting workup up by a specialist might provide more answers/treatments.

Treat for Dandruff : See Dermatologist.
Dandruff is associated with malassezia fungus infection.
Try over-the-counter dandruff shampoos, and look for one that contains zinc pyrithione, which can reduce the fungus; selenium sulfide, which can limit cell turnover and decrease the amount of fungus; salicylic acid, which works as a scrub to slough off dead skin; or ketoconazole, which works against a broad array of fungi.
