Go to Doctor: This is unusual and there is very little here to give you insight to.  I would recommend that you get in to see your podiatrist soon and let them get tot he bottom of this.

Easily treated.....Stop wearing those shoes!

Early or late.: It really depends on if they are new or established. Early wounds may be treated with Hydrocortisone to prevent post inflammatory hyperpigmentation if not infected. Older scars may benefit from tradional methods with resurfacing, peels, dermabrasion, laser, bleaching, or even excision.

Hydrocortisone: Cream or ointment.  Can also use a Benadryl (diphenhydramine) cream.  Ice packs are a natural way to help with itch.

It depends . . .: It depends on what you mean by "little bit" and what type of diet you are trying to follow.  If just cutting calories then certainly counting the calories of the ice cream you eat should be taken into consideration of your total daily allotment.  If on a low glycemic index or low carb diet, even a little bit can throw you off due to the sugars.

Neosporin: If the bite is infected, Neosporin may help temporarily until you can be seen by your physician. An truly infected roach bite should not be self-medicated.  Neosporin does not contain a pain reliever so it is not likely to help relieve your pain. If you are experiencing pain at the site of the bite, it could be a sign of infection which may require a prescribed antibiotic.

Itchy bite: The most effective topical treatment for the itch is a cortisone type cream or ointment. Use the cream on moist areas and ointment on dry areas. If there is associated spreading redness and warmth, secondary bacterial infection is likely and warrants an examination by a physician.
