Lots of things: Osteoarthritis is the most common cause, any other type of arthritis including rheumatoid arthritis, psoriatic arthritis, and gout can effect the toes.  Calluses, bunions, and ill fitting shoes can also contribute.  The doctor can usually identify these problems by examining your feet, but sometimes x-rays or labs are needed.

The second, third and fourth toes have two in each toe. The fifth or little toe can have either one or two. Some consider the joints that join the toes to the foot (metatarsal phalangeal joints) as toe joints also. Toe pain is when any one of these joints hurt.

Possible  causes: There are  possible causes  as  simple as  cysts from the joint, joint swelling or arthritis. Ask your physicians to have you see a rheumtologist

Hello. : Hello. I can't diagnose your symptoms. However, it does sound a lot like podagra which is when the base of the big toe contains uric acid crystals.  If you have not been evaluated for possible gout, recommend that you do so. In the mean time you might consider pain control through a non-steroidal anti-inflammatory medication like motrin.
