Yes: If "low iron count" is a pop term for anemia (low hemoglobin / hematocrit) and/or serum iron itself is low, this is always worrisome and your physician must either continue working you up or give you an explanation and suggested treatment. If you are having muscle pain, an elevated CPK would confirm but not prove a clinical impression of a myositis / rhabdomyolysis & lead to workup.

Get evaluated: This may be anything from muscular dystrophy to polymyositis / dermatomyositis to a recent injury. Severe exercise may rise CK into the thousands but usually not this high. Junior will get a workup by competent physicians. The best you can do is educate yourself about his condition when an accurate diagnosis is made, and be a good brother for him in a world that's often cruel.

Depends: CPKs are enzymes inside the cell that are release after injury to the membrane cause by trauma,inflammation,ischemia or even excessive exercise.It comes in different isoforms depending on the cells it comes from such as skeletal,heart,liver,brain etc. So it can be chronic as in inflammation of skeletal muscles or transient as in trauma or heart attacks or induce by drugs.
