Depending why you are on coumadin (warfarin).
It can be reversed within a couple of hours.

Probably nothing: Your Coumadin (warfarin) regimen might be altered before the surgery or there can be no changes.  At the present time, there appears to be a shift towards not altering the Coumadin (warfarin) levels.  If you need dental surgery, such as an extraction, then the oral surgeon does have the materials to help stop the bleeding.  Discuss any concerns with your physician and oral surgeon.

Stop before surgery: Coumadin (warfarin) should be discontinued for 4-5 days before a surgical procedure to avoid excessive surgical bleeding.  It takes that long for the effects to wear off. For patients on coumadin (warfarin) because of a mechanical heart valve, some form of heparin is sometimes prescribed for a day or two before the surgery and then stopped (heparin is stopped closer to surgery because its effects don't last as long.)

Many possibilities: Many potential reasons. I'll enumerate:blood loss during surgery; phlebotomy or in other words frequent in-hospital blood draws; anemia of chronic disease ie inability of bone marrow to stimulate and churn out normal volume of blood components in response to life stressors as it used to.

When : When they are very fresh--less than two weeks old--clots can often be dissolved or mechanically broken down using a variety of techniques.  However, these approaches are usually contraindicated in a patient who has recently had surgery.  With older clots, like yours, blood thinners like Coumadin (warfarin) are the usual approach.

A patient who develops a lot of clots in multiple areas of the body needs to be evaluated for an abnormality in the clotting system.  Have you been seen by a hematologist? If not, that would probably be an appropriate next step for you.

Do not waste $: Talk to your doctor about a sensible weight loss program. Green coffee beans are not the way to go despite testimonials.

i take a 150mg capsule twice a day.  i started this prescription on 11/17/2012 and prior to that i took 7.5mg coumadin

Consult your doc: This question should have been addressed by your surgeon in consultation with the doctor who put you on it. The proper answer also needs to take into account why you are on the pradaxa. You may need to take Heparin injections between the time you stop the Pradaxa and the surgery.
