Need exam: See a physician for a physical exam. May have tonsillitis, strep throat etc.  Regarding acid reflux, try over the counter antacids like Omeprazole daily. See you physician.

See a doctor now: Tuberculosis, sarcoidosis, and cancer of the lung, throat or upper GI tract are three main causes. There are other rarer ones but they aren't good either. Do not delay! have it evaluated this week.

Yes: Hi, topamax (topiramate) should not cause any change to your immunity.  However can cause dry mouth and nervousness that may exacerbate an underlying vocal tic (cough).  I would look for other causes of the cough, but the only way to be sure would be to come off the topamax (topiramate).

Not completely sure: Clonazepam is not typically associated with rapid weight loss when discontinuing it and I have not seen that,but it's possible it's happened in studies before.I do hope you tapered off the medication as abruptly stopping can cause major problems such as worsened anxiety,tremors,seizures and other mental problems.Bad dry cough is also not known to commonly cause weight loss unless you aren't eating

Rash &amp; weight loss: The fatigue and wight loss are more alarming and you should be evaluated for systemic diseases such as  malignancy or serious infections such as HIV of course if you have any risk factors for it ... Autoimmune diseases such as SLE, RA are other possibilities ...Rash can be associated with many diseases ... See your PCP soon !

Mgt: Children can have a number of colds in the course of one year, but the weight loss is excessive. Children at this age should gain weight steadily. 
Your physician may recommend a diet to include more calories and protein. In the meantime, if there is any respiratory difficulty such as air hunger, shortness of breath or wheezing, please see your ED at the hospital.
Watch for prolonged fever.

Have it evaluated: It is impossible to say without being able to do a physical exam and know the results of x rays and lab studies. The cough needs evaluation in a smoker because of the high risk of major lung issues. See a doctor soon.

It's accurate...: in theory some people may not have enough of an immune response to anything to be able to have a positive test. For this reason similar tests to other things people are likely to have a reaction to are included in a positive control called an anergy panel (it's never worked for me) and a chest x-ray is another good screen indicated in every chronic cough that always has.
