Tough to say: But the process of coughing n and of itself causes a pressure downward in the groin area.  That is why to test for a hernia a dr. Has you turn your head and "cough."  you may have strained your groin if the cough is severe and prolonged enough to cause an issue in the groin area.  Best wishes.

Testicular pain: Severe testicular pain can be caused by torsion, if pain of sudden onset. Also torsion of appendix testis. Epidiymitis due to inflammation causes severe pain. Epididymal cyst or spermatocele can be tender &amp; painful. Hemorrhage into testis tumor can be painful. Hydroceles &amp; varicoceles can cause mild pain. Engorgement of testicular veins so called "lovers nuts" or "blue balls" can be very painful.

Dr to examine testis: In general, testicle pain means a trip to the doctor right away because the testicle's blood supply can be twisted off (called torsion). What if only slight aches or pains? That would likely be some other, non-urgent problem. One's primary care doctor can help, because the details matter... where is the pain, how did it begin, how bad is the pain, how strong is the shower, is there a rash, etc...

Epididymitis: Is a medical condition characterized by discomfort or pain in of the epididymis, a curved structure at the back of the testicle in which sperm matures.  This can be painful, and should be evaluated and treated by a dr.  There can be other causes of pain in the testicle as well, including a torsion, where the blood supply can be affected and its truly an emergency.  Testes pain should be evaluated.

Torsion or Epid'itis: Acute testicular pain must be considered a twisted testicle until proven otherwise and demands emergency care to avoid loss of testis. Other sause of severe pain is epididymitis, an inflammation of epididymis and underlying testis, needs rest, ice pack and antibiotics. Other causes not serios other that hemorrhage into testis tumor. Scrotal ultrasound can differentiate, . Get seen asap. Good luck.

Unusual: Acute or classic genital herpes should not cause isolated testis discomfort.  Herpetic neuralgia can cause neuropathy and a host of symptoms.  Urologic evaluation and examination are important.   Please see a urologist.
