I assume you are : Asking why the cortisone injection made your back worse. Most times the injection helps the first few hours with pain relief due to local anesthetic and then a period of feeling worse due to the original pain PLUS the mild trauma induced by the needle, THEN feel better with cortisone the next 2 to 7 days. But do call your provider if you experience fever or excruciating pain after a few days.

Try Manipulation: Before resorting to steroid shots or systemic steroids, i would strongly suggest finding a do who specializes in treating low back pain with osteopathic manipulation. Many people have obtained adequate relief from omm to be able to take little or no drugs for their low back pain. It might not be the answer in every case, but it's worth a shot before resorting to steroids.

Psoosibly!: I you have true L5 or s1 pain without numbness, absolutely.  Numbess is next in likelyhood, and if you hve musclar weakness, or foot drop, even surgically prognosis is poorer. Paraxically, the most minimal nerve compression gets the external non-myelentated nerve fibers, ie the least compression.. But if you elect not to have surgery, i recommend schwinn -airdyne works and epidural blocks.

is it normal to have an ice cold foot with low back pain and sciatica?

Cold feet: Can happen with sciatica but must also think about peripheral vascular disease  and disorders involving sympathetic nerves etc if it persists.
