Spots: I have no idea what your doctor meant about black spots. That is not a terminology we use. You'll have to ask your doc about it.

: Too many symptoms for one doctor. The TMJ can be treated by the dentist. The clogged ears by an ENT specialist and the chest pains by a an internist. Your chest pains are not from your heart, but a strain or indigestion, stress and or anxiety. All your symptoms are overlapping and possibly related. It's two years now, don't complain and see a medical specialist.

Get tested: The constellation of symptoms as well as your environmental exposures in my opinion want a workup with the clinician. This way they can maybe can check your vitals and perform testing as indicated.

Costocondritis: Costochondritis is an inflammation of a rib or the cartilage connecting a rib. It is a common cause of chest pain. Did your doctors rule this out?  What was their opinion?

: I would hate to expose a kid to the type of contagion and trauma that comes through an ER. The event likely produced some musculoskeletal trauma  that may include rib injury. An urgent care center with x-ray capability or her regular doc would be a good venue. Cool compresses over painful areas will help. If she can't sleep, take her in.

Need to examine: It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated.  Wish you good health!

i woke up with upper chest pain the past two days. it does not hurt when i am in a neutral position but if i push my shoulders either back of foreword?

Musculoskeletal: You pain sounds musculoskeletal.  There are joints, bones, and muscles in the chest and they all move when you move your chest.  If any one of those is injured or inflamed, it will hurt.  It is unusual that musculoskeletal chest pain is of a serious nature and with time, it will improve.
