i have bradachardia, 1st degree av block & congenital heart disease papvr. is clarithromycin safe to take? it causes heart problems? no?

Very specific proble: Clarithromycin can cause prolongation of the corrected QT interval (measurement on EKG). If overly prolonged, dangerous rhythm disturbances can result (torsades de pointe ventricular tachycardia). It will not affect or be affected by sinus bradycardia, first degree AV block, or partial anomalous pulmonary venous return. If your QTc is normal, then it's safe for you to use it.

why do patients with an av block 3rd degree have a negative t wave on bipolar ekg?

Abnormal Rhythm: 3rd degree av block = complete av block... Meaning the upper chambers are not in synch with the ventricles. The ventricular depolarization is abnormal, the t-wave which are basically the re-set waves, will be abnormal.  Many times the t wave is flipped.  This isn't unexpected. The ventricular beats will be junctional or ventricular escape beats and the t-waves will vary.

: Males get a pass on much of the effects of their bad habits. They make millions of sperm from stem cells &amp; the healthiest of that bunch swim better to do the job. Women get the bad deal since they are born with all the eggs they will ever have &amp; anything they do in life or during pregnancy may effect the outcome. I wouldn't worry about it, don't smoke after baby comes if you want it to be healthy

Depends: Aortic insufficiency or aortic stenosis can cause diastolic congestive heart failure if not treated properly.  Depending on the extent of the condition, medication, or surgical options are the standard of care.

First degree AV bloc: First degree AV block can progress over time to higher degrees of AV block.  It does not always progress however to higher degrees of AV block. It is associated with overall higher mortality/morbidity risk than normal AV conduction even if progression of the AV block is not common.

Electrical vs fluid: Imagine a pump with loose wiring from the switch to the motor. Could there be a delay (after turning on the switch) before the pump starts working, but the pump works fine? That's the analogy. 1st degree AV block is an electrical delay on EKG from the switch (upper atria) to the motor (lower ventricular chambers). It says nothing about how the blood moves, which is in the nuclear stress images.

Not uncommon: It is not unusual to see it, particularly on 24 hour ekgs (holter monitor) during sleep. It is usually not a problem, but each person is considered individually.
