Need work up: You need to be evaluated in person by a physician for adequate treatment to begin. That could include a neck xray,  palpation for trigger pains in the neck muscles,  shoulder and arm muscles as well as checking for carpal tunnel syndrome.   The doctor may try trigger point injections,  a referral to physical therapy and possibly an MRI of the neck.

Doubtful: Tingling and numbness are not typical symptoms of tmd. I would recommend that you see a dentist to address all your dental issues and evaluate your tmjoints, etc. I am assuming that you are under the care of a physician for the polyneuropathy who can address the medical issues.

Neck pain: Yes that position could cause impingement of a nerve to produce your symptoms. You need a complete history and physical examination done and probably imagining of your neck.

Head trauma: The symptoms are consistant with mild head trauma.  He should be OK with rest and being careful.  If he gets worse instead of better, then see the doctor.

A bad or increasing headache; recurrent, persistent, or weird headache; or with other worrisome symptoms needs a doctor's exam.
