No: No, it is non-specific.  Chest pain from gi, gastrointestinal, conditions is also relieved with nitroglycerine at times.  For example, esophageal spasm.  If you are experiencing this pain, you must seek medical attention to evaluate your situation.

Nitroglycerin: Nitroglycerin relieves  cardiac related chest pain by dilating the coronary arteries. These are the arteries that supply blood to the heart. Improving blood flow to heart muscle will relieve the discomfort caused by a partial blockage of the blood vessel with plaque.

Lying down: It is surprising to me that a 23 year old would have significant enough coronary artery disease to require these medications. Nitroglycerin will briefly lower your blood pressure more which could be detrimental. If you are lying down when you take it you may not pass out or feel as dizzy. I think you need to confirm the need for these medications with your health care provider.

Not necessarily: Nitroglycerin is a vascular smooth muscle relaxant. It is indicated for the treatment of coronary arterial disease. Some patients benefit from sublingual Nitroglycerin for acute symptoms of esophageal spasm as well. Symptoms of esophageal spasm mimics angina. Chest pain from heart is usually related to exertion and activity last no more than 5-10 min and improves with rest.

Nothing: Nothing would happen. People who aren't used to nitro sometimes get a headache that is harmless and goes away over about 30 minutes. People who overdose will get low blood pressure and can pass out. There are no serious consequence.  It is often used in practice by people who have chest pain of unclear cause with no harm done.
