Possible: Depending upon the time of injury, it should show with swelling or redness besides the local tenderness that you describe if it had accompanying fractire or dislocation. A plain Xray will be helpful as an initial study at an urgent car center and an arm sling for comfort as indicated. You may benefit from PT, which may require a referral. Feel better!

Chest pain: At 61, male with cholesterol, hypertension and sleep apnea I recommend seeing a cardiologist. Although your labs have been normal they do not help to define your problem. You should get a stress test and an evaluation by a specialist who can define your problem.

Need to pin down: Symptoms in arms are not related to a disk in the lower back, although one might experience disk issues in neck also.  Best though to wonder about peripheral nerve disorder, spinal stenosis, and even medication reactions.  Need a thorough evaluation by a neurologist.

MRI/MRA: The Magnetic Resonance Imaging test is best test for brain tissue, but the MRA important too as it looks at the blood vessels in the brain also. headaches are complicated, and usually difficult to find association with an imaging study. This is why most Doctors do not order imaging of the head for headaches, unless it is the worse headache of your life. I suggest looking at a more complete picture
