Crowns and fillings: For someone your age this is a lot of dental work.  You need to get serious about your  oral care.  Control your diet and reduce sugars, sodas, sugary and acidic drinks.  Start brushing and flossing, stop smoking and see your dentist for cleanings as they recommend.

Shouldn't Be at all: With today's anesthetics, you should feel anything when getting the cavity filled.  You will feel as brief "pinch" whilst the anesthesia is adminstered.

Depends: Depends on where and how large the cavities are.  Is the dentist filling the teeth also doing the braces for you?  If not, I am sure he/she will consult with orthodontist

It shouldn't: But an infection can. See your dentist to check those teeth.

Pain from cavity: Pain after a cavity is filled shouldn't be severe. There might be some gum irritation for a day or two that would probably respond to over the counter Ibuprofen or a mild narcotic such as tramadol or low dose vicodin. If the pain is severe you should call your dentist and get back for another examination.

At the hospital: Midazolam is safe when the child is being monitored.  Sedation in a child may be unexpectedly problematic.  In my view, pediatric sedation should only be performed in the hospital with full monitoring and emergency staff and equipment.

Sometimes: It is usually best to diagnose using the aid of x-rays, but it is not always necessary.

Not the filling, but: The actually filling should not, but sometimes the bonding matrial that is excess does, or a piece that doesn't belong called flash comes off.  Check for rough spots, if you don't find one it likely was flash or excess bonding material.  Ask dentist if needed.
