Maybe fungus?: "white spots" can certainly be a yeast infection in the skin but can be many other problems.  Dermasone is a steroid and can make any kind of infection worse.  If that occurs see your doctor.

If she has had nipple/breast enlargement this would be part of the sequence. When the anus drains it smells like poop. The nearby location can make it confusing.
