my 3 weeks old son has swelling in his right nipple, is that cause for concern? what should we do?

Usually normal: It's normal for babies (boys and girls) to have mild or even swollen, enlarged breasts and/or lumps under the nipple. Almost always benign and due to exposure to maternal hormones in the womb, same hormones that cause the mother's breasts to swell and  produce milk.
Over weeks to months,  no more exposure to the hormones, the breast tissue begins to shrink and eventually flattens.

Boy w/swollen nipple: Some boys going through puberty develop breast swelling which can be painful. This is normal and usually disappears within several months. Cold compress over the involved breast can help ease the pain.

hi-i have a slight swelling of a vein/duct just above one of my nipples. it is painful to touch and reddish. i can feel a long swollen ridge. any idea what caused this?

Be examined: this could represent a supericial infection or simply tissue inflammation without infection or possible some more significant entity.   I suggest that you be examined by a physician  to determine the exact cause of this area of discomfort.

Infection?: It sounds like there may be an infection or abscess. I would all and make an appointment to be evaluated and possible antibiotics, ASAP.

Get checked: an infected implant can be a big problem. see your plastic surgeon for an exam asap.

Nipple pain: If you think you might be pregnant check then check a home pregnancy test. Other causes of nipple pain are weight gain, weight loss, hormonal fluctuations, caffeine intake, changes in diet, changes in exercise patterns. ill fitting bras, skin disorders, breast masses and many others. Schedule a visit for an examination to help sort out the many possible changes.

what are possible causes of unilateral, non cyclic, dull ache behind nipple with slight breast swelling. no palpable masses per gyn. mammo/sono 2mro.

Could be normal: Without a palpable mass, this is unlikely to be malignancy or cancer, but it is good to get imaging to rule this out.  Other possibilites include muscle/skeletal injury, infection, pregnancy, lactation, hormonal anomaly, and other.

Possibly an Insect--: bite, seems the most plausible explanation; other would  include a trauma to the affected site. Both these would cause other symptoms as well.
Suggest you see your PCP for a detailed History and an Examination and possible need for investigations; All these elements would be required to provide you with the answers you seek.

Fat or low protein: Could be just fat. Or low protein causing you to retain water.
