If the child is 4 years old, and the rash is just a few days, in the hands, the feet and the mouth, then it would be cause by a virus, the condition is called hand-foot-mouth disease. Several associations: Rashes can be caused by many things including infections.  Was the rash on the girls palms?  This may be an indication she has syphilis.  Its actually rather uncommon for rashes to be on peoples hands in regards to infectious disease.  You need to have comprehensive blood work including RPR and titre, HIV testing and use a condom when having sex.

Unlikely: There are few things that produce a rash on the palms and soles, including syphilis and tick borne illnesses like rocky mountain spotted fever.  This sounds like you should go to a doctor and be evaluated.  Good luck.

See allergist: The hives are not related to your Rheumatic fever.  Consult an allergist to find out the reason.  Wow, I have not seen a case of rheumatic heart disease since my internship year decades ago.

Wouldn't be acne: Your 2 yo can easily have bumps on his face triggered by illness, reaction to food or something that came in contact. They do not get acne. Without seeing this and reviewing the kids history little can be offered as a reason. If it persists have it looked at.
