In some cases it occurs in the abdomen or pelvis and grows to an enormous size at which time it can be painful.  Even in the skin of the head and neck at times the lesion has some mild tenderness.  In the presence of lymphedema it is the Stewart Treves syndrome representing a lymphangiosarcoma.
