This varies a lot, but often it is like a localized bruise that doesn't last long and goes away with rest on its own.
Mostly, it just occurs but can be after trauma

i had open gallbladder removed  out over a year ago but now my stomach is big. i have a fat pad in there and hate it. is this caused from surgery?

Unrelated: probably you are not eating before  due to gall bladder attacks , now you  are free , no longer has pain , eating better ( more ) , additional factor lack of exercise . If it is the cause modify your life style , be more active with exercises and healthy balanced diet.

an injury caused a large joint effusion w/ elevation of the anterior & posterior fat pads on elbow.  will the fat pads heal and return to its place?

Kind of: The fat pads are normal structures and so they don't really heal.  In a x-ray of a normal elbow, they are usually not seen as they are within the joint.  When the elbow joint has extra fluid in it, they float out of the joint and are visible.  So when you see fat pads on an x-ray, that is a sign of a significant elbow injury even if a fracture is not seen, as in an occult radial head fracture.

bump size of my palm just under c6 vertebrae. am slightly overweight so thought fat pad or or dowagers hump coz i look at phone, causes no pain or discomfort. been there for about 3 years and hasnt changed?

: Probably a lipoma ( fat cell that has grown into a lump). No reason to worry but should have your primary md look at confirm

Patella: chondromalacia is a disorder of the ANTERIOR =FRONT Compartement of the knee and by itself does not usually cause pain BEHIND the knee!!

NO: So called Hoffa fat pad  syndrome is felt in the FRONT of the knee! You could still be having pain from your recent surgery..check your knee out with your Orthopedic Surgeon!
(Sounds like an EFFUSION "water on the knee"
