Blood loss: Blood loss from the gastro intestinal tract is the most common cause. This could be due to gastritis, ulcer, hemorrhoids, diverticuli, or tumor. Nutritional deficiencies can lead to anemia as well. Cancer of the blood is another concern. Seek medical attention promptly.

Menstrual blood loss: In women the commonest cause of iron deficiency anemia is excessive blood loss through menstruation. It can generally be made up by taking oral iron supplement and a multivitamin. It would be good idea to see a doctor to exclude other causes.

Shoulder: Or abdominal pain is not a symptom of anemia.  Causes of shoulder and abdominal pain are so many that i cannot possibly cover here.  A massively enlarged spleen can cause anemia, left shoulder pain, and abdominal pain, but this is a rare condition.

Iron def anemia: The cause of iron deficiency must be identified, especially in adults.  In her case, the likely cause could be her menorrhagia.  She should see her GYN to address this underlying cause.  All oral iron preparations are equally effective.  IV iron may be considered if the patient has ongoing blood loss, impaired oral iron absorption or intolerable GI side effects.
