No: I'm assuming these pricey capsules actually contain what they claim to. If so, they're harmless and you are paying for something you could get much cheaper as whey powder or steak. It's your money.

Many possible causes: The testicles can atrophy for any number of reasons. Hormonal causes include underproduction or impaired response to androgenic hormones & antiandrogenic medications. Vascular problems such as testicular torsion or varicocele can lead to atrophy. Severe infections of the testis, such as mumps virus or bacterial infections such as epididymo-orchitis can lead to atrophic scarring.

Explain: If you mean that you had hydrocele surgery then it happened check that side testicle with the other if equal nothing is wrong. If smaller than it may be related to surgery- or not. check with your surgeon

Many things.: Acute scrotal pain can be caused by testicular torsion, infections such as epididymitis, orchitis or mumps, appendix testis torsion, cancer, hernia, hydrocele, spermatocele,  varicocele & traumatic injury.  If you have sudden pain, please see a physician since torsion or infection must be treated immediately. A physician will be able to assess the need for scrotal ultrasound & direct treatment.

Varicocele's Ache: Yes. The swelling of the varicocele leads to pain. The ache is usually short lived as the trauma that led to the inflammation subsides.  There is a surgical option if the pain is intolerable. But, active varicocele's can hurt.
