Failure: To clean under the gums. The bacteria left behind create acids which destroy the bone supporting the teeth. It is easily cured with treatment and if you follow all instructions on home care it will not come back and you will have your teeth until you do not need them.

Bulimia : Bulimia generally does not "cause" periodontal disease.. But it can cause many other oral problems, , including gindival irritation, and chemical burns (due to chronic contact with stomach acids) .. Also, the vey same issue (stomach acid in the mouth)  can severely  destruct the internal/lingual surfaces of the teeth,  causing major  damages to the integrity  of the tooth structure as well..

Wait-2nd trimester: I would advise waiting until your second trimester of pregnancy before getting any dental work done.  Unless there is an emergency.  Otherwise, you should be fine.  If you wait until later in your final weeks, you may not be comfortable in the laying position and having a joyful bowling ball sitting on your bladder etc.
Talk to your gp and the periodontist.

Dentist/Periodontist: Your periodontist or family dentist can do the dna testing and identify those. The best preventive measure for periodontitis (gum disease) is removing the bacteria colonies from your teeth and gums especially in between teeth. Use soft tooth brush and floss twice a day. Concentrate on one tooth at a time, take your time. At first you will have some bleeding & soreness due to the infection.

Not well known: I do not know of any well-established correlation between smoking marijuana and incidence of periodontal disease.

Depending on the: Severity / extent and duration of the periodontal disease / infection it would seem possible to have an elevated esr.  Best to be evaluated by a dentist to rule out dental / periodontal etiology.  Hope this helps.

Reverse gingivitis: Gingivitis is the gum swelling associated with poor home dental care. If left untreated it will progress to periodontitis which means that you are losing alveolar bone and ligament attachment. If you get treatment for the gum disease coupled with improve brushing and flossing technique and habit, you will see the gingivitis go away. Bone and ligament loss will stop but not reverse itself.

Inflammatory Disease: in the periodontal pocket may cause tissue destruction.

Depends: If the spacing is recent, it would indicate changes in the bone support or occlusion. Best to schedule a dental visit for xrays and probing to determine cause.
