Anatomical: You might try looser underwear, but some people have a shorter gubernaculum testis - than others - this is a ligament that holds the testis like a tether....  If the pain get severe or redness /tenderness occur  - you should definitely see your doctor - sometimes the testes can tortion or twist and become very painful - and this is a serious medical issue.. Myth: The sperm you have is constantly created and shed. If you don't use it in partner or self sex, it leaks out in the night as a spontaneous emission. You don't get "blue balls" from retention you get it from getting excited with partner petting that stops before release. Even that will subside if you cool off.

No risk, no worry: Blood borne infections and STDs are not transmitted by such contact. The infection risks from this sort of contact are colds, incluenza, and some intestinal infections -- but none of the infections you mention (except maybe hepatitis A on rare occasions).

Rash: Possible allergy, although, without seeing a pic and getting more detailed story - it's no more than pure guess. Try Benadryl (diphenhydramine) 25 mg at night and see if it helps. Avoid drying brands of soap (usually bar soap) and hot water (making itching worse). If no better after the Benadryl (diphenhydramine) or other symptoms are present or the rash getting worse, see doc.
