Not likely.: All women are generally considered fertile.  About 25 % of women will become pregnant with unprotected sex each month 75 % will not.  You play the odds every time you have unprotected sex. 

If you do not want to be pregnant but fail to use some method of protection there is indeed something wrong.

Less: Less likely, but certainly possible.  Actually for many men the preejaculate or clear discharge that moistens the tip of the penis can have a high concentration of sperm.  So although he may not have ejaculated inside. He can deliver enough sperm to cause pregnancy without "finishing".

Yes: If the guy inserts his penis in your vagina, either pre-cum or ejaculate can impregnate you.  Unless you are ready to be pregnant, birth control is necessary.  Condoms will protect against pregnancy and std's.
