Very rarely: It takes a bit of force to produce a concussion-so while in a rare circumstance one might turn the head fast enough(elderly) or rotate fast enough(e.G spinning car or amusemnt park ride) this would be uncommon.

Wouldn't think so: Your body has fail-safes in its function that prevent such an injury by simple head shaking. I suppose if you spent hours doing it you could eventually cause injury, but it would likely be to muscles rather than brain.

If you are having symptoms you may benefit from seeing your doctor, treatment is normal brain rest. 90% of people get completely back to normal in less that one week.
