Yes: I do not have any direct concerns for caffeine and an SSRI.  I would generally recommend use of caffeine in moderation regardless and lowering if possible if concern for anxiety.

Yes but....: Yes , you can but you should be careful because there is a greater risk of side effects.
Provigil and caffeine  are both wakefulness promoting agents, so  it is possible that the stimulatory effects of the two drugs will be  increased when taken together. Use caffeine and other stimulants like energy drink etc with caution.

No interactions: There are no known interactions between caffeine and Strattera. The FDA cites 400 mg a day as the recommended limit of caffeine for healthy adults. The American Academy of Pediatrics discourages caffeine for children. Consider decaf soda if at all. After 33 hours: The half-life of Cipro (ciprofloxacin) is about 4–6 hours in subjects with normal renal function is approximately 4 hours. Heavy daily caffeine intake, more than 500 - 600 mg per day may cause  side effects. Thus, 400 mg is a maximum daily dose = 4 caps. Take care.

Not sure.: The answer depends mostly on what kind of supplement you are taking and why you are taking it. Especially if the supplements contain some sort of stimulant, it may be a bad idea to combine the two.

Not much.: Some antibiotics may slow down clearance of caffeine from your body, but that means you drink coffee less often, not give it up completely.
