See below: Ms. Kathryn 440: You have two related questions. It would be better to talk to your doctor for explanation. Blood HCG needs to be higher than 25 to diagnose pregnancy. It is not clear how a urine pregnancy test told you that you are 2-3 weeks pregnant. You may repeat the urine pregnancy test, be sure to follow the instructions for the test carefully. Better would be to see your doctor for a repeat blood test and if needed an ultrasound examination.

Often the woman's: Body handles this on it's own. If the woman is having significant problems or bleeding or it isn't resolving then surgical intervention (d & c) may be required.

Yes: You may have a miscarriage without ever knowing it.  You may have a heavier than normal period or some cramping followed by bleeding.  Please consult your physician for proper birth control and testing for STD's.  Unfortunately I diagnose STD's in young people your age on a regular basis. Yes.: Missed abortion or blighted ovum is a condition where your pregnancy or embryo is not growing or not formed. Some women will not know and not have any bleeding or pain. It is diagnosed on an ultrasound.

Varies: Sometimes it is obvious, many times it is not.  Most women have some amount of bleeding during early pregnancy due to implantation and they may think they have (or are) miscarried.  The best way to tell for sure is with an ultrasound and  sometimes by following serial HCG levels in the blood.
