YES: All of the above symptoms can be caused by a large ovarian cyst.  This should be followed and reported to your physician. Twisting of the ovary on its blood supply can cause these symptoms and if not addressed can cause  serious problem and possibe loss of the ovary

Secondly, a cyst that size is usually removed to rule out malignancy, You need to consult with your gyne; what did the u/s said about the morphology

1-14 days: Generally when a cyst ruptures, the ovary starts producing progesterone. Progesterone can cause some tiredness, but not usually extreme fatigue.

Next Step: It is unclear what would have lead to testing a Ca-125 in a 26 year old female.  Now that it has been demosntrated to be elevated  in the setting of ovarian cycsts further evaluation should take place.  The likley next step would be pelvic imaging with an US to urther characterize the cysts.  It should be noted that ca-125 is not specific and can elevated for many reasons, not just ovarian cancer.

Depends: It may be twisting or ruptured. You should see your doctor and have an ultrasound done. The headache and fatigue are probably independent symptoms.

Talk to your surgeon: You need to visit with your GYN surgeon and ask him/her that question. Usually we request that you minimize vigorous exercise and activity to help prevent twisting(torsion) or rupture of the ovarian cyst.

Chronic Pelvic Pain: If chronic, usually is part of the pain syndrome in the title. Need to eliminate structural lumbar abnormalities. Diagnostic work-up usually unremarkable.

Hormones: Some cystic issues in the ovaries may cause hormonal changes...which in turn can possibly mimic some symptoms of a pregnancy.
