Exam: It is possible that your IUD has shifted but an exam would be necessary in order to know for sure and would be an opportunity to look for other causes if this isn't the reason you are having the bleeding and your partner had pain in his penis with intercourse. Hope this helps.

Typically : Typically allergic reactions are immediate, a delayed hypersensitivity (allergy) reaction usually occurs 3 - 7 days after exposure; therefore the likelihood of you having an allergic reaction to Mirena is slim.  People who are allergic to levonorgestrel, or to any other progestins (norethindrone, desogestrel) should not use mirena. Also Mirena may contain inactive ingredients which can cause allergic reactions; however like i stated previously this would have occurred a lot sooner. 
Since you have seen many doctors and have not found a cause you may consider having the Mirena removed and monitor for improvement.

Avoid: Avoid hot water bathing. Take zyrtec daily or allegra (fexofenadine).

: It is not feasible to provide a meaningful opinion without additional history, and physical examination. You may watch and wait. If the swelling more than doubles, you develop an ulcer or discharge, inguinal lymph nodes become enlarged, you get fever or lose weight, see a doctor.

The "distribution": of the rash you describe sounds like the area covered by your underpants! Look for allergy to your LAUNDRY SOAP.  Since that "area" is also usually a warm/possibly sweaty  place a FUNGAL skin infection is another possibility.

: They could be tiny hemangiomas (dilatation of blood vessels) which are totally harmless. But for precise diagnosis you have to be seen by a Urologist.

Fungal infection/balanititis, herpes, warts, syphilis are examples of infectious processes your have aquired somewhere/somehow. It is difficult to tell what you have based on your question.
