$100-$1000 : The cost of an extraction cannot really be determined until you get an xray of the tooth.  The cost will then be determined upon how hard it will be to remove the tooth.   If the tooth is broken or not erupted all the way, the cost will be more.  Your dentist will take an xray then discuss the cost with you.  Don't forget to add in the additional cost of pain medication after the extraction.

Extractions: Your dentist will numb the area so you don't feel anything. Then she\he will remove it with special dental instruments. At that point you will be give post-op instructions. Then you will have a space where the tooth was removed. Most likely, you will need to have that tooth replaced. Ask all your questions to the dentist removing the tooth. Good luck.

Usually no: It depends on how loose the tooth is, how much root structure is present and how heavily they are attached to the gums.  Local anesthetics can be used to remove the tooth.

If there is an: Abscess and/or pain involved and this was a recommendation by your dentist or pedodontist, then yes.  An oral surgeon will be able to comfortably and safely sedate your child for this procedure - so that it will not turn into a "bad experience" and lasting negative impression for them.
