Usually, no: If you already have herpes, having sex with someone else with herpes shouldn't cause you to have an outbreak.  The outbreak probably would've happened regardless if you had sex or not.  The only situation i can imagine this happening is if the other person had a different type (type 1 vs 2) than you.

Herpes type I ?: Herpes types I and II are usually self limited ulcers which are painful but do not generally cause "horrible back pain".  Shingles, however, which comes from Herpes Simplex (latent "chicken pox") can cause very painful skin lesions and lower back pain. See your physician for testing to be certain.

not often: Simple rashes with a virus are common. Hives are not. They do come up occasionally, but I would look to a food allergy or other incidental exposure as more likely.
