40yo caucasian male, blood pressure at 130/90, urine test seems ok. low bone mineral density, blood tests for bone and kidney health were normal, except for low calcitriol 15pg/ml and pth at 70pg/ml (then 40pg/ml after two weeks). any ideas?

: The blood pressure reading is higher than I would want anyone to have as baseline. For most people, there is no defined origin to elevated blood pressure. BP elevations are stealth killers, leading to organ damage over decades. I would discuss your findings with your doc and were it me I would get on meds.

To a point...: In general an increase in bone mineral density is a good thing for older adults. There are some unusual conditions (paget's disease) where one can have too much bone.  If you have a very bone mineral density already there is probably no an advantage to getting it even higher.

It depends: Bone scans are used to evaluate for bone infections, recent fractures, and bone cancer, and are not used to check for osteoporosis.  Most people never need one.  Bone density testing should start when there is a reasonable probability of increased fracture risk that would require treatment.  So it depends on risk factors.  A common age to start would be 50 for women or 70 for men.

Listen to your body: Vit d3 is deficient, parathyroid is excessive and bone density is decreased and calcium is on the  high side of normal. Phosphate intake and blood levels may be increased causing parathyroid levels to be high. Magnesium may also be deficient. Re-evaluate life style. Eat 6 healthy meals with 6 servings of fresh green vegetables, get sufficient vit d3 from sun or supplements, avoid colas.

why would u do a bone density test on a woman my wife is geting one done

To : To see if she if her calcium levels in the bone are affected. In women, if estrogen levels drop (could be for a variety of reasons, one including menopause) calcium levels drop too, causing drop in bone density, fractures, brittle bones and possibly bone deformities. Bone density test sometimes needs to be repeated more than once to show actual drop in bone density.

Yes: Bone density tests measure the amount of calcium in the bone to determine a person's risk for fractures.  Bone scans use a radioactive tracer to identify areas of abnormally high  cellular activity in the bone.  Some conditions that may show up on a bone scan include recent fractures, infections of the bone, or some forms of cancer in the bone.

DXA: A bone densitometry test, or DXA, is fast and easy. Yes, they weight you. It's very useful information, and can allow intervention to reduce fracture risk before a fracture occurs.

Accepted Answer

Yes: Depends on level of instrumentation on whether your spine will be able to be tested and compared to previous.  Hips will still be able to be tested in order to risk stratify you.
