Yes: Hey!  you are a human being.  If you got your thumb split, would it hurt?  Cutting into tissue and through skin hurts.  Is that really a mystery?

How bad?: If you have a short lingual frenulum the tongue could appear slightly split.  If you cut your tongue slightly, it will generally heal on it's own just fine.

The cost depends: I assume you are referring to splitting the tongue down the middle (i.E "forking" the tongue)?  This is a relatively unusual request, but I have performed this on several patients.  The cost depends on whether you want to have it under local anesthesia (cost ranges from $500-2000) or under iv/general anesthesia ($1500-5000).  Insurance does not pay for it.  It can be reversed.

Yes: The tongue is highly inervated and vascular.  There are many potential complications to this surgery and the surgeon should be licensed and knowledgable about the complications.   Pain is managed like other soft tissue surgeries in the mouth, with pain medications and antibiotics.

Split tongue: I am assuming you mean there is a fissure
in the middle of your tongue

it has always been that way and your tongue is
of normal appearance.....It is what it is

if this split is new and you have not seen it before......See an oral surgeon.....

Trauma: Did you bite the tip to cause it to split?
Not a hugh deal if split, some people are having this done to look liked a snake tongue..
