I'm : I'm sorry to hear you are having these symptoms.
After a multi-level laminectomy it is possible that you have developed spinal instability in the lumbar spine and if this is the case a fusion is one option, but not the only option.

I treat conditions such as yours on a regular basis and would be happy to examine you and review your imaging studies in order to discuss your condition and what options are available.

Generally a spinal fusion is reserved for instability or spinal deformity. Special tests can be ordered to determine if this is the best approach, or if a more conservative approach is appropriate. Age in of itself is not an indication or contra-indication for fusion, however, taking into account that you are 18, it is imperative that the proper work up is implemented prior to moving forward with any type of fusion procedure.

Depends on reason: This depends on the  reason for this type of surgery, the number of levels involved. , whether this is a repeat surgery, the health and age of the patient, the smoking status of the patient, their weight and nutritional status and other co morbidities or problens like diabetes, thyroid disease, etc.. The bone density can also play a role, ie, osteoporosis.

They should but wont: Work comp is terrible about authorizing treatment including surgery. The congenital narrow canal is not work comp related but since you have narrow canal, you are likely to suffer earlier and more from any degenerative condition that narrows the canal more like disc protrusion, bone spurs, and ligament over growth. This can all arise from cumulative work comp related injuries.

Gradual worsening: Gradual worsening with foot drops if you have significant neurologic symptoms peotecting the nervesc with reconstruction has a better long term out com if you have only axial backache surgical outcome is less obviously beneficial wiltze urged us to wait for the leg pain then intervene if you dont have leg pain then your nerves are ok.

3-anesthetic injections, known as nerve blocks.

Failed back syndrome: Failed back syndrome (FBS) can be caused by many different factors. Laminectomies, in particular, have been shown to result in spinal instability in biomechanical studies. Minimally invasive surgery (mis) can help obviate some of the complications associated with fbs. Unfortunately, there may not be a quick fix in your particular situation. I would obtain a formal consult from a spine specialist.

is a laminectomy or minimally invasive foramenectomy w/o fusion better for mixed foraminal  and spinal cervical stenosis?

Ask your surgeon(s): They'll review your imaging with you and discuss your options. But before further surgeries, you mention physical therapy and cymbalta, (duloxetine) but there may be other non-surgical options including other medications, pain psychology and interventional pain procedures that may be helpful for your condition.
