Need more work up: None of the things you mention should be related to eye pain in my experience.  See an ophthalmologist to explore things that could be related to the eye pain part, but you really need a reason for the swollen spleen, testicular pain and weight loss more than just a ct scan and blood work.  Every city has what i call the "zebra" doctor.  We have a saying that the thunder of hooves should be horses.

Not common, but yes: Sciatica is a condition where the sciatic nerve is irritated. The nerve is made up of branches from the 4th lumbar nerve (l4) down to the 3rd sacral nerve (s3). Scrotal and testicular sensation are typically provided by the s2 (racepinephrine) nerve. There are other more common causes of testicular pain, but disc herniation in the back can potentially cause this pain.

Yes, but get seen. : Disc herniations have been known to cause chronic testicular pain, called orchalgia. If you are experiencing testicular pain, be sure to get evaluated. There are many causes, and it is important not to simply attribute it to your lower back issues but workup any cause that may need to be addressed.
