Local or systemic: Could be related to local disease process like sinus or nasal infection or more a more systemic process such as flu or other infection, dehydration etc.  Imuran would predispose you to infections.

Fluid: Ear effusion at times?  If no fever and not persistent likely cause allergies.  You may want to try antihistamine.

Normal: It would be a normal sensation to get light-headed or dizzy after performing a valsalva maneuver for several seconds.  A valsalva maneuver is when one has a forceful expiration against a closed mouth and nose.

Chronic fatigue: Teenage female with chronic fatigue &amp; intermittent tinnitus &amp; dizziness needs a general examination for such problems as anemia, dysautonomia, Lyme, hormone dysregulation, fibromyalgia and other disorders. If syncope or fever are involved, then there is more urgent need for an appointment.

Migraine: You could be suffering from migraine headaches. You should see your physician. Over the counter paracetamol or ibuprofen could help alleviate the pain but not prevent reoccurrence.

Common : That antibiotic can cause  dizziness, so it could be associated with that rx. If symptoms persist or worsen, contact your doctor for possible rx change.
