i have an embarrasing issue- sometimes in the shower i noticed severe burning pain if i accidently scratch the scrotal skin.thepain lasts for a couple of minutes and then dissipates.there  is no swelling or redness. what can it be?

Scrotal sensitivity: The scrotal region is highly innervated by sensory nerve endings which can detect the slightest tactile sensation and therefore, a scratch may overly stimulate densely packed nerve endings and be interpreted as painful especially if there were hot water coming onto the area at the same time from the shower.  My guess is that the same situation outside the shower doesn't bother nearly as much.

Likely perirectal ab: If it's only going on a few days and getting worse sounds like a perirectal abscess, if so should be drained sometime over the next 24 hour, can usually be done in the er.

Prostate also : Sounds like you already are being treated if you know that it is a e coli infections   prob also a prostate infection and should improve with proper antibiotics.

Connection: The testis are formed near the kidneys & migrate down into the scrotum over time. During descent they leave the abdomen in a tubular projection of a lining of the abdominal wall called the peritoneum, which adheres to the testis & blood vessels like shrink wrap. If the shrink wrap doesn't shrink enough, clear abdominal fluid can end up in the scrotum next to the testis.

No: Cocaine can cause sperm damage and infertility but not painful swelling. It sounds like you have an infection. Either epididymitis or orchitis, either way you need to see a doctor for diagnosis and treatment.

Lots of boys + men: Hydroceles are most common cause for scrotal sweeling in boys. Collection of fluid. Resolves spontaeously by age one year in most infants unless huge. Beyond on year + if fluctuate in size or in adults need surgery to clear. Varicoceles + epididymal cysts are very common in post pubertal males. Testis torsion and epididimitis common + present as painful swellings which need treatment. Tumors rare.
