i hit my elbow hard a few yrs ago and have a hard bump on it now that feels like bone, what could it be?

Scar tissue: Probably developed some scar tissue. Should get an xray to make sure.

Cyst: it is probably a sebaceous cyst or a lipoma and is of no consequence but can easily be removed if it bothers you

if my daughter chipped the tip of her elbow when she dislocated it would the bone fragment need to be removed?still has lots of pain and a small bump

Not that likely: Dislocated elbow in an eight year old is very uncommon but not unheard of /dislocation of the elbow is a very serious injury 
tip of the elbow is the olecranon  and usually not involved directly in a dislocation
if you are concerned you must talk with your treating surgeon or seek another opinion.

hi. my symptoms are: swollen lymph nodes behind ears, head, neck, collar bone, and throat (under jaw). no pain & appeared 2 days ago. no cold/sinus/fever. this am noticed hot coin-shaped rash on buttock & itchy bumps on elbow. 6 weeks pregnant, 39yrs?

Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. It would be prudent to see your doctor. Being that you are pregnant makes it doubly important that you see a doctor, promptly.

Be seen: I definitely would not hesitate to be seen.  In that area, the most common cause would likely be a swollen lymph node, which can often come before or after a febrile illness.  Although it would be unusual in your age group, I would not hesitate to be seen by a general surgeon if it does not resolve to make sure it is not a neoplastic process.

Rashes are tough: Rashes are tough to diagnose without history or pictures. A common rash under the nose is impetigo caused by Staph. Consider a video consult so that we can look at it.

Bump: Inside elbow is a common place for eczema and skin irritation, try moiturizer.

Reaction: It appears that you may have a reaction to albuterol when you take it via a dermatologic manifestation.  Trying some antihistamines may reduce the presence and occurrence of this.

Rash: Any skin condition is difficult to identify without seeing it or having more history. What does it look like?Is it red?Flad/raised? Etc. It could be that your knees have come into contact with something and is showing an allergic reaction. Could be eczema or psoriasis or skin rashes can sometimes be a sign of something going on internally. Best to have a doctor take a look and take a full history.
