Often myofascial: If this is spontaneous neck pain, no injury, it is likely myofascial - pain in the soft tissues of the neck.  Benign condition.  Often treated with heat, ice and OTC medications.  Stretching often helps.  If it persists, have it examined by a physician

Muscle spasms: It could be spasms of the muscles on the left side of your neck.  I would take some ibuprofen and consider seeing a chiropractor for treatment.

Relax: Anxiety or stress may lead to fatigue and tension in the back of the neck. Try to relax. Go for a walk, get fresh air, progressive muscle relaxation or deep breathing exercises. Take care.

Infected cyst??: Sounds like you might have had a sebaceous cyst which got infected, hence it is harder and tender now. These are benign swellings in the skin occurring near a hair. 
See your doc/surgeon about it for proper DX and treatment.

Lump DDx: The differential diagnosis (DDx) of "lump" is mass-ive. If resolving with heat may be a tight "knotted" muscle. Consider seeing your primary MD to help with history and physical exam.

have chronic pain in back of neck and up and over the head ..both sides and forehead . scalp can be tender .. been 1 1/2 years ... have tried phys. therapy with only temp. minor relief .. am i doomed with this pain forever i am so frustrated .i am 72?

: Problem has persisted for almost 2 years so I assume you had had full work up for the etiology. What is the presumed diagnosis? Besides physical therapy, have you received medication(s) for chronic pain such as antidepressant med +/- pregablin/gabapentin because the pain seems consistent with neuropathic pain. If doctor has tried everything without success, you should consider acupuncture.

dx costochondritis now tender over entire rib cage front&back of neck scalp breast and jaw stiffness. mild relief with opiod and nsaids is this normal?

No narcotics: if you are not getting relief, I suggest calling your prescribing physician and after stopping opioids and NSAIDS and ask about the use of a steroid dose pack ( Medrol (methylprednisolone) or prednisone ). Narcotics will not cure the inflammation.

Get it checked...: We have only few glands in the back of neck and swelling/lumps you feel are often from sebaceous cyst, which if infected can cause pain and may look like the one in picture here. Lymphnodes are typically small and mobile but can get larger and sore if there is infection near-by the area. If it is bothering you, get it checked. Boil can be lanced and may need antibiotic. Consult doc. Good luck.
